Nonconformity detected – what should be done next?

During the implementation of any international management system standard (such as ISO 9001, ISO 14001, ISO 45001), it is natural and inevitable to identify nonconformities during internal and external audits, as well as in daily process controls. However, the main issue is not only detecting the nonconformity but ensuring its effective elimination and preventing its recurrence.

If a nonconformity is detected, your next step is to manage it and implement corrective actions. These stages cover identifying the nonconformity, conducting root cause analysis, preparing an action plan, execution, and documentation.

1. Describe the nonconformity clearly
The first step is to document the essence of the nonconformity clearly. This should precisely indicate the location, cause, related processes, and impact. A clear description solves half of the problem.

2. Document the nonconformity
When a nonconformity is detected, facts must be recorded accurately and communicated to the relevant parties. The information gathered at the initial stage serves as key evidence for later investigation of the causes.

3. Conduct root cause analysis
It is important to identify not just the symptoms of the problem but its actual root cause. Tools such as the “5 Whys” or the “Fishbone Diagram” can be used. Without finding the root cause, corrective action will only be a temporary solution and may lead to higher costs in the future.

4. Prepare a corrective action plan
Corrective actions should not only solve the problem but also prevent it from recurring. The plan should clearly define responsible persons, actions to be taken, and timelines.

5. Implementation and monitoring
Once approved, the plan should be executed and results monitored. Effectiveness can be verified through testing, re-audit, or performance measurements.

6. Documentation and continual improvement
Documenting each step creates valuable experience for managing similar situations in the future. Once the nonconformity is resolved and verified, update the records and close the nonconformity report.
